Achievement Awards Forms and Licking County 4-H Committee Scholarship

Calling all 4-H members who will be 4-H ages 14-18 in 2020! It’s time to complete your 4-H Achievement Awards Forms. What is an achievement awards form? It’s a bit like your 4-H resume: a summary of your involvement and accomplishments throughout your 4-H career. By filling out and turning in your awards forms, you will be eligible to win special awards, trips and opportunities through the county and state 4-H programs. The top winner in the state for each project category is invited to attend the National Club Congress, which is the highest honor a 4-H member can receive. We’ve had many Licking County 4-H members attend, will the next one be YOU?

It can be a little overwhelming the first (and maybe even the second) time you fill out your awards forms. To help with that, there is a workshop coming up that can help you better understand the process.

Wednesday, September 18, 2019 – 6:00pm – Licking County Extension Office – RSVP by September 16 – Informational Flyer

This workshop is geared toward those who have never done awards forms before or those who would like help improving upon what they’ve done in the past.

All forms (both PDF and Word Documents) can be found on the Member Resources page here. Completed forms are due to the Extension Office by 4:30pm on Friday, October 18, 2019! Forms MUST have your advisor’s signature.

For those members who are currently high school seniors or higher and were actively enrolled in 4-H in Licking County for 2019, you are eligible to apply for the Licking County 4-H Committee Scholarship. Please review the application here for a full list of requirements. Please note: you must complete both the Achievement Awards Form and the Scholarship Application in order to be eligible for the scholarship.

For questions about the Achievement Awards Forms, please contact Lisa McCutcheon at mccutcheon.46@osu.edu.

Teen Interest Inventory

If you are a Licking County 4-H member who will be 4-H age 14-18 in 2020, you should have received an email that invited you to complete the Teen Interest Inventory. In case the email didn’t make it to you, here is another chance for you to complete the Teen Interest Inventory.

We have many exciting opportunities available specifically for teens for the 2020 4-H year. Since most clubs don’t meet much, if at all, during the fall and winter, it can be easy to miss deadlines. To help you keep up on these opportunities, we ask that you please complete the Teen Interest Inventory. By completing the Teen Interest Inventory, it let’s us know which opportunities that you are interested in so that we can send you the information specific to those opportunities when it becomes available. For instance, if you mark that you are interested in becoming a camp counselor, we will send you a link to the application along with other information once it becomes available so that you don’t miss the chance to apply! This interest inventory is not a commitment to applying for the opportunities you select, it is just a way to let us know to send you more information.

In the past, we’ve sent out a paper that we asked to fill out and send back in. To make it easier this year, we’re doing an online survey. You can easily fill it out in just a few minutes from a computer or mobile device and you don’t have to worry about getting the form back to us!

The teen opportunities are listed below. Once you have reviewed them and know which ones you are interested in, please follow this link to complete the survey: https://osu.az1.qualtrics.com/jfe/form/SV_efGY3vSXCfvJuIdPlease complete the survey before it closes on Monday, September 30, 2019.
