As previously announced, a new tax form, Form 1095-C, will be provided to faculty and staff as part of Ohio State’s reporting requirements under the Affordable Care Act (ACA). Faculty and staff who consented to receive the form electronically will be able to download the form by February 15. For those who did not provide consent, a hard copy form will be mailed to the home address by the end of February.
