Information for Qualtrics users at OSU Wexner Medical Center
Due to the sensitive data handled by employees of the OSU Wexner Medical Center, please make sure to review the requirements and restrictions below.
To upgrade a Trial account to a standard account that provides access to the full capabilities of Qualtrics (such as distributing surveys and unlimited questions), please visit the Getting Started page for more information.
Requirement:
- Completion of HIPAA and Institutional Data Compliance FYxx Training (current fiscal year) in BuckeyeLearn.

Additional Restrictions:
Sending surveys to specific populations may be subject to additional approval. Some examples include, but are not limited to:
- Surveys involving human subjects research (per the U.S. Department of Health and Human Services [DHHS] Code of Federal Regulations definition), an APPROVED Institutional Review Board (IRB) protocol must be in place before distributing the survey.
- Surveys going to the OSU Community must meet accessibility standards before distribution.
- Surveys going to nursing personnel should be approved by the Ohio Nursing Association representative. This includes surveys conducted by OSU nursing faculty, staff, and students.
- Surveys going to patients or their families/caregivers must be reviewed by the Patient Experience Team if it is being distributed for OSUWMC use or Medical Center approval is required, and it has not been approved through another process.
- Surveys going out on behalf of The Ohio State University Wexner Medical Center will need review and a signature from Communications & Marketing prior to distribution.
Surveys that do not require these approvals may still be subject to additional general guidelines in certain situations. Be sure you are familiar with our Best Practices in Compliance with Survey Research.
