If possible, in Section 3 select one disease, like an enteric or vaccine preventable disease such as measles, where your LHD has had experience or has investigated a potential case. Create a screenshot of that page along with the downloaded file with reporting information. 4. Include any investigation policy or procedure from your health department based on the disease selected in #3.
Option Part B: Ohio Public Health Lead Investigation Manual (PHLI) 1. Download PHLI Manual and submit along with a screenshot that lists the appendices. 2. Also include any documents that demonstrate procedures at the local level. |
