CME Webcast Teaches on Transitioning from Pediatric to Adult Care

CME Webcast Teaches on Transitioning from Pediatric to Adult Care. This webcast will discuss Transitioning from Pediatric to Adult Care. The transition from pediatric to adult health care is a complex process, particularly for adolescents and young adults with chronic health conditions and intellectual and developmental disabilities. In this presentation, learners will expand their understanding of the transition process, with particular attention on the roles of doctors caring for young adults in the transition process and get an introduction to skills and resources that can support young adults as they enter adult health care.

By the mid-20th century, medical advances have led to improved survival rates for conditions that we once considered fatal in childhood, such as cystic fibrosis and congenital heart disease. As a result, the need for structured transition programs between pediatric care to adult care has become increasingly evident. Organizations such as the American Academy of Pediatrics, the American Academy of Family Physicians, the American College of Physicians and more have all emphasized the importance of transition planning to ensure continued, high-quality care. So, what does this mean for us in practice? How can we support our patients through this transition? To help us understand why transition of care is so critical and answer these questions and more, we’ve invited Dr Laura Hart. Dr Hart is a clinician-researcher at Nationwide Children’s Hospital where she practices Med-Peds primary care and researches the transition from pediatric to adult centered care.

Learn Latest on Sickle Cell Disease on Webcast

OSU Wexner Medical Center logo

Learn Latest on Sickle Cell Disease on Webcast. This program will go through the challenges for people with sickle cell disease to obtain primary care, especially the challenges around the transition from pediatric to adult care. Learners will also learn about preventative, acute, and chronic care topics important for primary care physicians caring for individuals with sickle cell disease. The second half of the program will go over the pathophysiology of sickle cell disease, disease modifying treatments, and management of chronic pain.

For this webcast, we’ve invited two experts to discuss modern care of patients with Sickle Cell Disease with an emphasis on primary care. First, Dr. Rob Cronin is an Associate Professor of Internal Medicine & Pediatrics and is a clinician scientist who not only provides primary care for patients with sickle cell disease.  He also uses his clinical informatics training to study topics including sickle cell disease, disparities, and transitions from pediatric to adult centered care. Our second presenter is Dr. Andrew Schamess. Andrew is an Associate Professor of Internal Medicine who has helped create a novel home-based primary care program for patients with sickle cell disease.

What you’ll learn in this webcast

As a result of this educational activity, webcast participants will be able to:

  • Describe preventive care for people with SCD, including nuances for this population
  • Describe some common acute and chronic issues in SCD that intersect with primary care and how to manage them
  • Identify three disease modifying agents that are used to treat sickle cell disease.
  • Identify three factors that can contribute to chronic pain in adults with sickle cell disease.

CME Webcast Teaches on Cardiac Risk Assessment

OSU Wexner Medical Center logo

CME Webcast Teaches on Cardiac Risk Assessment.  This talk will focus on a review of guideline-based risk assessment for atherosclerotic cardiovascular disease. We will define risk severity for both primary and secondary prevention with the help of some risk calculator tools. We will define risk-enhancing factors for cardiovascular disease and their appropriate utilization, with special focus on coronary artery calcium score and lipoprotein(a).

Back in 1948, there were only a couple of fairly ineffective antihypertensive drugs and there were no drugs to treat high cholesterol. But today, we have extremely effective medications to treat both high blood pressure and high cholesterol in order to reduce the chances of cardiovascular disease. But which patients should be treated and how aggressively should they be treated? Fortunately, we have solid evidence-based answers to those questions and today, we are going to explore the tools that every primary care provider can use for every adult patient to prevent cardiovascular disease. We’re pleased to welcome cardiologist and Assistant Professor of Internal Medicine, Dr. Benjamin Romer.

What you’ll learn in this webcast

As a result of this educational activity, webcast participants will be able to:

Understand guideline-based risk assessment for atherosclerotic cardiovascular disease

Be able to utilize risk calculators and understand risk-enhancing factors to best define risk

You’ll also learn about the following:

  • When to begin cardiovascular risk assessment in your patients
  • Harnessing the electronic medical record for cardiovascular risk assessment
  • Use of cardiovascular risk assessment for medication de-escalation
  • Statin dosing
  • Lipoprotein(a) testing
  • When to order Hs-CRP
  • Treatment of elevated levels of Hs-CRP
  • Role of the routine EKG in cardiovascular risk assessment
