CME Webcast Teaches on Transitioning from Pediatric to Adult Care. This webcast will discuss Transitioning from Pediatric to Adult Care. The transition from pediatric to adult health care is a complex process, particularly for adolescents and young adults with chronic health conditions and intellectual and developmental disabilities. In this presentation, learners will expand their understanding of the transition process, with particular attention on the roles of doctors caring for young adults in the transition process and get an introduction to skills and resources that can support young adults as they enter adult health care.
By the mid-20th century, medical advances have led to improved survival rates for conditions that we once considered fatal in childhood, such as cystic fibrosis and congenital heart disease. As a result, the need for structured transition programs between pediatric care to adult care has become increasingly evident. Organizations such as the American Academy of Pediatrics, the American Academy of Family Physicians, the American College of Physicians and more have all emphasized the importance of transition planning to ensure continued, high-quality care. So, what does this mean for us in practice? How can we support our patients through this transition? To help us understand why transition of care is so critical and answer these questions and more, we’ve invited Dr Laura Hart. Dr Hart is a clinician-researcher at Nationwide Children’s Hospital where she practices Med-Peds primary care and researches the transition from pediatric to adult centered care.
