YP4H Biometric Screenings (faculty and staff only)

You MUST be preregistered for this event.

It is important that you Know Your Numbers.  By knowing your height, weight, BMI, blood pressure, total cholesterol, HDL, and blood sugar (either glucose or A1C) values, you can make changes to improve your health and reduce your risk of developing heart disease, diabetes and other serious illnesses.   Beginning in 2018, the biometric values that you can use to earn the premium credit must be verified by a health care provider such as an on campus screening or by a PCP.  This can be completed in one of the following 3 ways:

  1. On-Campus ScreeningEvent

o   On Sept. 7th,  the OSU Health Plan’s RN Health Promotion Specialist(s) will be in our department to complete complimentary screenings. These values will automatically load to your YP4H/Virgin Pulse account on the Stats section. See below for registration details

 

  1. OSU or Central Ohio Primary Care (COPC) Provider Visit

o   Your values obtained during an annual exam with an OSU or COPC provider will automatically load to your YP4H/Virgin Pulse account on the Stats section.

 

  1. Non OSU or non COPC provider 

o   If you had a physical or completed a biometric screening with your PCP after December 1, 2017, you can request that your provider submit them to the OSU Health Plan using this formhttps://yp4h.osu.edu/wp-content/uploads/2018-Provider-Fax-Form.pdf

 

Registration Details:

If you would like to register for the upcoming biometric health screening in our department, follow the steps below:

  1. Visit yp4h.osu.eduand click the “Schedule a Screening” button in the middle of the page.

 

  1. Log in with your Ohio State name.# and password.
  2. Highlight “BiometricHealth Screening” and find the “appointment code” box.
  3. In the “Appointment Code” box typein PH2018   Do NOT copy and paste as this doesn’t always work to activate the code.
  4. Click “Find Appointments or Classes”- This will show only the appointments available for this screeningevent.
  5. Continue to schedule the appointment.  If you experience any difficulty, please contact Customer Service at 292-4700 with your Appointment Code.

Once you obtain your required health values, complete the Personal Health and Well-being Assessment (PHA).  The PHA is an online self-evaluation of your overall wellness and only takes about 15 minutes to complete.  After completing the PHA, Virgin Pulse will offer specific programs to you and customize your YP4H experience based on your goals.  Visit yp4h.osu.edu and log in to Virgin Pulse to complete your PHA.

By annually completing the PHA and a verified biometric screening, enrolled employees can reduce the cost of their medical contributions by $20 per month or by $30 per month if they and their enrolled spouse/same-sex domestic partner both complete the PHA with verified biometrics. Please remember that participating in the wellness program is completely voluntary. We understand that your health history is very personal — protecting your information and keeping it private is of utmost importance to us. While participating in YP4H, the personal information that you provide, whether online or in person, is only available to you and those whom you authorize to access it. Beyond the team at the OSU Health Plan, your nurse, health coach, or your own PCP, no other Ohio State personnel will see or have access to your results.

Research Opportunity – Fan Health Network

Leverage the latest in wearable fitness technology and expertise of legendary athletes/sports personalities by participating in a pilot study to test the effects of the Fan Health Network (FHN) intervention. Faculty and staff who decide to participate in this research will engage in an 8 week healthy behavior challenge, using a new exercise and healthy habits app. Earn FHN points for participating in exercise, reading wellness articles and social media activities, and qualify for a raffle to win incentives. Joining the challenge is simple. Please visit https://go.osu.edu/fanhealthnetworkpilot to complete the participant consent and survey. Then follow the instructions to download and register for the Fan Health Network app from your smartphone. The program begins August 12!

A Message from the Chief Wellness Officer 
Hello everyone,

Summer is a great time to build relations with friends and family. Fostering connections with others creates in us a sense of belonging that is so important to optimal social wellness.

Social wellness is not merely having a big group of friends and family with whom to connect. It includes developing the skills that will help you relate to others in a healthy and meaningful way. There’s a great deal of social wellness that is learned behavior. We can all continually improve on these skills.

Feelings of loneliness or disconnect can take a toll on your body. If you’re feeling socially disconnected, here are ways to build your support network and foster social well-being:

  • Disconnect to connect – Disconnect from technology as a daily routine to spend some time communicating face to face with people at home and work.
  • Connect with your community – Volunteering can give you a sense of self-worth and connect you to other people. Read more. 
  • Be positive – With a positive outlook, you will enjoy yourself more, and other people will enjoy being with you.
  • Build a culture of respect – When you model respect for others who may be different from you, you gain respect, too. Read more.
