- All immunizations, titers, and TB testing must be complete prior to the start of the program, regardless of when your clinical rotations begin.
- All of these requirements are submitted to Wilce Student Health Services via MyBuckMD or by emailing preventivemedicine@osu.edu . If submitting via email, please include your full name, date of birth, and OSU ID number for easier processing.
- If you need to be re-vaccinated for Hepatitis B because the first series does not provide immunity, please contact Wilce Student Health Center regarding a timeline for becoming re-vaccinated. As long as you are following this timeline and submitting the appropriate documentation, you will be considered temporarily compliant until the series is completed.
BCI and FBI Background Checks

- Please see this link for specific instructions for local and out of state students. You do not need to come to Ohio to complete this requirement.
- If you have completed your background check and are concerned your results have not yet been update in compliance records please click here for troubleshooting tips.
- Both a BCI and FBI Background Check must be completed prior to program start. We are unable to use a background check from a previous employer, even if that employer was part of the Ohio State Wexner Center. You will need to complete a new background check for this requirement.
- Please use reason code ORC 4273.09 as the reason for finger printing services.

- BCI and FBI results can come in at different times, and this does not necessarily indicate an issue. These are two separate processes and successful completion and receipt of one, does not indicate the status of the other.
- Out of state fingerprinting will take longer than in-state printing, this does not necessarily indicate an issue.
- It is the student’s responsibility to ensure that the background checks arrive at the College of Nursing prior to all program compliance deadlines.
- You will only be marked as compliant once both your BCI and FBI are received and cleared. We are not able to update compliance status based on receipts from the purchase of the background checks.
CPR Certification

- The College of Nursing only accepts BLS for the Health Care Provider or BLS Provider from the American Heart Association.
- Please have the front and back images of the student’s CPR card on one page in PDF format. This certification expires every 2 years and must be updated with the College before the expiration date to remain compliant.

Drug Screenings:
Completed First 1 to 2 weeks of program

- All College of Nursing Traditional Masters students are required to complete a drug screen.
- Full time students will be screened in November of the autumn semester. Part time students will be screened in January of spring semester of their first year.
- Students will be assigned a specific day in which the student will be required to complete a drug screen at the Wilce Student Health Center.
- The cost of the drug screen will be covered by the College of Nursing only if it is completed at the Student Health Services Laboratory.
- ** Please bring prescriptions of all medications/supplements currently being taken in the case of a false positive result. **
- If you are an Online Distance Student you may have your drug screen completed at another location, but the cost will not be covered by the College of Nursing. Please see these directions in order to be screened for the correct substances. Insufficient panels will not be accepted.
HIPAA:

All College of Nursing students are required annually to complete HIPAA certification. This must be submitted each year when the updated module is released.
1. Login to https://buckeyelearn.osu.edu
2. Click “My Transcript” button and locate the HIPAA training and click “Launch”
If you are unable to find the training, please contact hughes.1561@osu.edu. For all other support, please email 8help@osu.edu or call (614) 688-4357.
3. No further action is required once you have completed the HIPAA training. Your compliance status will be updated within 24 hours after completing the training.
Confidentiality agreement

This form is to be signed by all students and faculty accessing patient information or providing patient care. This may require access to computerized patient record information such as IHIS (Integrated Healthcare Information System); One Source; Pyxis medication access; getting patient care supplies and equipment accessed through Omnicell; bar-coded ID access to specialized units or areas such as Women & Infant and entrance to secure medication and patient care supply areas for individual units. Release of information authorization form

The Ohio State University College of Nursing clinical sites are obtained throughout Franklin and contiguous counties. When the OSU Medical Center (Wexner Medical Center, Ross Heart Hospital, James Cancer Hospital, Harding Hospital and OSU East Hospital) the OhioHealth System (Riverside Methodist Hospital, Grant Medical Center, Doctor’s Hospital, Grady Memorial and Dublin Methodist Hospital) and Mount Carmel Hospital System (Mount Carmel West, Mount Carmel East and Mount Carmel St. Ann’s) are utilized it is a necessity for students to have access to computer systems used for clinical care. If a student does not have access to the computer system, the student will not be able to meet the requirements of the clinical course and may be at risk of failing the course. The above hospital systems where the college of nursing places a student for clinical experiences require that a unique identifier such as a social security number or ITIN (individual tax identification number) be used for each student in order to create access to these systems. OSU Medical Center also requires date of birth. Signing this form grants The Ohio State University College of Nursing permission to release this information to any of the above institutions in the event you are placed in one of these facilities.
