Flu Vaccine Deadline
The flu vaccine is available and the deadline to get vaccinated was November 1st

- Flu vaccine documentation must be submitted through the Enterprise Health Portal and facultycompliance@osu.edu by November 1.
- If you received your flu vaccine at Employee Health
- Log in to https://osu.enterprise.health with your osu credentials
- Click on ‘Immunization History’ from the column on the left
- Save as a PDF and submit to facultycompliance@osu.edu
- If you received your flu vaccine at a location other than Employee Health
- Documentation of flu vaccine is being processed through the Enterprise Health portal. This information must be entered there by the employee themselves. Please navigate to https://osu.enterprise.health to begin. You must also submit documentation to facultycompliance@osu.edu.
This page describes compliance requirements for faculty residing in the state of Ohio. If you do not reside in the state of Ohio please reference the out-of-state faculty instructions page.
Faculty can view their compliance status by logging in to https://records.nursing.osu.edu/. From the menu ‘Compliance’ click on ‘Your Faculty Compliance Report’. Any items that require your attention will be listed next to ‘Reasoning’.

Please view your compliance status to determine which items listed below are required for you to complete.
Below you will find the meaning of flags that drive compliance requirements:
- No Contact – ‘Yes’ indicates no direct patient contact including no oversight of students in clinical experiences. Those without this flag are required to meet compliance requirements consistent with potential for patient contact in their faculty role.
- Research Involvement – ‘Yes’ indicates participation in research and scholarship
- APRN – ‘Yes’ indicates use of an APRN title requiring submission of an Ohio APRN license
- CPR Exemption – ‘Yes’ indicates exemption due to medical/physical reasons
- TB Special Unit – ‘Yes’ indicates working in an identified high-risk unit and must receive TB test annually

Immunizations and Health Data
Submission Instructions: all medical documentation and health data should be sent to Employee Health via the Enterprise Health Portal for processing.

- All local faculty must go to Employee Health to become vaccinated. For hours of operation please visit the Employee Health website: here
- TB Requirement
- All new hires will receive a QFT. Click here for more information about the QFT blood test.
- Faculty in high-risk units are required to receive the TB test annually.
- High-risk units were identified by the OSU Wexner Medical Center and include the following: Clinical Microbiology (AFB lab and processors); Ear, Nose and Throat; Infectious Diseases; Pulmonary; Pulmonary Diagnostic Lab; Respiratory Therapy (main medical center campus and east hospital).
- All other faculty must complete the Infection Prevention Starts With You FY25 training annually. Please follow these instructions to access the Infection Prevention training:

- Login to https://buckeyelearn.osu.edu.
- Click “My Transcript” button and locate the Infection Prevention training and click “Launch”. Choose the “Provider” option to complete the training. If you do not see the training, you will need to search for it in the top search box.
- No further action is required once you have completed the Infection Prevention training. Your compliance status will be updated within 24 hours after completing the training. If you need assistance searching for the Infection Prevention module, please contact Beny Walujo at walujo.1@osu.edu.

Toxicology Screen
Submission Instructions: must be completed at Employee Health
- Non nursing faculty are not required to complete a toxicology screen. For clarification on whether you are considered non-nursing faculty please contact Amy Colson in HR.
- Faculty will be given instructions upon hire with how and where to complete their toxicology screening.
APRN Licensure
Submission Instructions: submitted via upload here
- Upload evidence of your Ohio APRN license including your full name, license number, and expiration date. The screenshot of your license can be taken from the Board of Nursing website.
CPR
Submission Instructions: submitted via upload here.
- The College of Nursing only accepts BLS Provider from the American Heart Association.
- Please have the front image of the faculty’s CPR card in one document. This certification expires every 2 years and must be updated with the College before the expiration date to remain compliant.
- Follow this link to find a BLS course in your area.
HIPAA
Submission Instructions: automatically submitted upon completion.
All College of Nursing faculty are required annually to complete HIPAA certification. Please follow these instructions to access the HIPAA training:
- Login to https://buckeyelearn.osu.edu .
- Click “My Transcript” button and locate the HIPAA and Institutional Data Compliance FY25 training and click “Launch”.
- No further action is required once you have completed the HIPAA training. Your compliance status will be updated within 24 hours after completing the training.

FERPA (self-enroll)
Submission Instructions: automatically submitted upon completion.
The title of FERPA training in BuckeyeLearn is The Family Educational Rights and Privacy Act at Ohio State. Completion of training related to FERPA is required of all who work with student data and must be completed annually.
Requirements: Ohio State lastname.# and associated password, and an active employee affiliation.
Please follow these instructions to access the FERPA training. The training may take up to 1.5 hours to complete. You do not have to finish all of the training at once. You can return to it by accessing BuckeyeLearn and selecting the MY TRANSCRIPT button in the middle of the BuckeyeLearn homepage. Please note, if you are unable to launch the course, you may need to disable your web browser’s pop-up blocker, please see go.osu.edu/blpopup for directions.

- Self-enroll in the course by selecting https://go.osu.edu/getferpa.
Since most users have completed FERPA last year, BuckeyeLearn may not automatically register the training. Digital Accessibility Policy Training

The digital accessibility policy establishes minimum standards for the accessibility of Ohio State’s digital information and services, whether created by Ohio State or purchased for our use, as well as the reporting requirements necessary to ensure compliance with the university’s obligations under applicable state and federal laws and regulations.
Please follow these instructions to access the Digital Accessibility Policy training:
- Login to https://buckeyelearn.osu.edu/.
- Click “My Transcript” button and locate the Digital Accessibility Policy 2024 training and click “Launch”.
- No further action is required once you have completed the Digital Accessibility Policy training. Your compliance status will be updated within 24 hours after completing the training.
If the Digital Accessibility Policy training isn’t listed on your transcript, please contact Beny Walujo at walujo.1@osu.edu.
Bloodborne Pathogens Training (self-enroll)

As of 2025, the title of this training is Bloodborne Pathogens 2025
This training is an introduction to the OSHA Bloodborne Pathogen Standard, sharps safety, common bloodborne pathogens and the University Control Plan. This training is offered online and is required annually for all employees who are at risk for occupational exposure to human blood, body fluids, cell lines or other potentially infectious material (as defined by the OSHA standard).
Please follow these instructions to access the Bloodborne Pathogens training.
- Login to https://buckeyelearn.osu.edu.
- Use the search field at the top right of the page. Use the term “Bloodborne“.

- Click on the “Bloodborne Pathogens” from the list (as shown above). It will open the training detail page and from the right side menu:
- If this is your first time completing the course, Click “Launch”.
- If you have completed this training before, Click the arrow on the gray box and choose “Request”. Note: this step is important because BuckeyeLearn will register the training and update the completion date properly.
- No further action is required once you have completed the Bloodborne Pathogens training. Your compliance status will be updated within 24 hours after completing the training.

Intellectual Property Agreement

All faculty, students, and staff working on federally-funded projects must sign the written Intellectual Property Agreement (http://go.osu.edu/ipa-form).
The agreement requirement applies to paid and unpaid research work, so students working on federally-funded projects must complete the agreement whether they are paid or working as part of course credit / research practicum / training plan.
Please follow this link: http://go.osu.edu/ipa-form to complete the Intellectual Property Agreement at your earliest opportunity if you have not yet done so and upload a PDF image of the signed agreement via the qualtrics link above. The agreement is a one-time request.
