Instructions for Frequently Used Data Sets

Access to most SHARP datasets requires a project request (link TBD) and completion of all necessary contractual agreementsm, such as identification of an analyst with SHARP Server Access, and a fully executed Data Use Agreement.  Some datasets require additional documentation.

For HCUP NATIONAL Databases: NEDS, NRD, KID, NIS

  1. Complete HCUP DUA Training, at https://www.hcup-us.ahrq.gov/DUA/dua/index.html
  2. Review and sign DUA for Nationwide Databases: https://www.hcup-us.ahrq.gov/team/NationwideDUA.pdf
  3. Ask Dr. Opalek to “invite” you to submit your signed DUA through the HCUP account.
  4. Submit your HCUP DUA, and wait for confirmation that it has been accepted by the HCUP site.

For HCUP STATE Databases: SID, SASD, SEDD

  1. Complete HCUP DUA Training, at https://www.hcup-us.ahrq.gov/DUA/dua/index.html
  2. Review and sign DUA for State Databases: https://www.hcup-us.ahrq.gov/team/StateDUA.pdf
  3. Ask Dr. Opalek to “invite” you to submit your signed DUA through the HCUP account.
  4. Submit your HCUP DUA, and wait for confirmation that it has been accepted by the HCUP site.

 

NOTE: Intended Use of HCUP State Databases:

      All requests for State Databases require a “Statement of Intended Use.”  If you do not provie all of the information required, the processing of your request will be delayed as the AHRQ reviewers seek additional clarification.

     Each application for and approval of HCUP data is project-specific. If you have previously purchased HCUP State data and wish to use the data for a purpose other than that originally approved, you must submit a “Data Re-use Request” to the HCUP Central Distributor for review and approval by AHRQ before work may begin on the new project.

 

NSQIP

The Ohio State University is a NSQIP-participating institution.

  1. Visit https://www.facs.org/quality-programs/acs-nsqip/participant-use/puf-form and complete the form.
  2. Click “Request Data File”
  3. Complete the Participant Use Data File questions.  Click submit.
  4. You should receive an email from NSQIP regarding your access request.   Please forward this confirmation request and a copy of your signed DUA to judy.opalek@osumc.edu.

SEER

  1. Complete the form at: https://seer.cancer.gov/seertrack/data/request/
  2. You should receive an email from SEER, with a confirmation link.  Click the link.
  3. Download, print and sign the Data Use Agreement form from the page you linked to, above.
  4. Scan the DUA form and send via email to seerfax@imsweb.com, and please cc judy.opalek@osumc.edu.
  5. You should receive another email from SEER confirming your access request. Please forward this email to judy.opalek@osumc.edu as well.
