Streamline Your BuckeyeLearn Tasks with Forms

We offer multiple forms to help you complete BuckeyeLearn tasks quickly and efficiently. Once submitted, your form goes directly to BuckeyeLearn Support, who will review your request and follow up via email. If additional information is needed, or if the task has been completed, you’ll be notified promptly.

Note: These forms are for university employees only. Medical center employees, please submit an eServices ticket for assistance.

Available Forms:

• Learning Assignment Request Form
  Purpose: Request for training to be assigned to one or more learners.
• Compliance Report Request Form
  Purpose: Request existing compliance reports or a new custom BuckeyeLearn report.
• ILT Location Link Request Form
  Purpose: Request the creation of new in-person classroom locations for instructor-led training (ILTs) in BuckeyeLearn.
• Security Role Request Form
  Purpose: Request BuckeyeLearn administrator access for an employee in your unit.
• BuckeyeLearn Playlist Creation Request Form
  Purpose: Request the creation of a curated Playlist using LinkedIn Learning content. Learn more in our Playlists blog post.
• BuckeyeLearn SCORM File Upload Request Form
  Purpose: Request the upload of a new or updated online course (eLearning) to BuckeyeLearn.
