SERVICE OUTREACH
If you are pursuing a service outreach opportunity that is not coordinated through STEM EE Scholars or our regular service partners, you need to complete the following form. It details service approval, the date/time of service, and the signature of the supervisor. You are still responsible for logging your hours on the appropriate form. An original, hard copy should be returned to the program manager no later than the last day of finals each semester. You are encouraged to keep a digital or hard copy for your files.

TRAVEL WAIVER
If you are participating in an off campus Scholars Event, you need to complete the following waiver. It should also be found in the emailed details of the event.
