Overview

The community assessment presentation will be a PowerPoint slide show that you will present via VoiceThread. Your presentation will highlight the results of your community assessment over the semester.

Objectives

This assignment measures your achievement of several RN to BSN program outcomes:

• 1. Integrate knowledge from a liberal education, the sciences, and nursing to address the health care of individuals, families, groups, communities, and populations across the life span in diverse and global health care systems and environments.
• 2. Apply a systematic process for application and evaluation of scientific evidence related to the principles of health promotion, risk reduction, disease prevention, and illness management across the continuum of dynamic healthcare environments.
• 3. Apply the knowledge and skills of information management and patient care technologies to deliver high quality nursing care that addresses legal, ethical, historical, and emerging issues.
• Communicate effectively and work collaboratively with individuals, families, groups, communities, and populations and within nursing and with other health disciplines to design, manage, and deliver high quality and safe patient-centered care.

Directions 

1. Create a presentation using 4-8 Power Point slides that includes the following information:
   • An introduction of your topic
     • Your chosen community and why
     • Main findings from windshield survey and demographic/epidemiology assessment
   • Problem Statement
     • Description of problem with problem statement
     • Rationale for problem chosen
     • SMART objective
   • Intervention used to address the problem
     • Evidence to support intervention (cited and referenced)
     • Rationale for intervention chosen
     • Resources needed for the intervention
     • How does intervention meet your SMART goal
   • Evaluation / Analysis / Synthesis
     • How will the intervention above (not SMART goal) be evaluated
     • How could you use this project
     • How could this project make an impact on the community
   • Provide a statement of your personal and professional growth with this project
   • Citations and associated references (in APA format)*
2. Use VoiceThread to record a presentation that includes no more than 7 minutes of audio discussing your community assessment.
3. Upload your presentation.

In the Week 14 Module, you will review two community assessment presentations submitted by your peers and provide feedback on VoiceThread.
