Hello,

This is a voluntary research survey with questions designed to examine the levels of producers’ awareness and knowledge regarding Foot and Mouth Disease (FMD) preparedness and response.  Please complete this survey by November, 15th.

The data collected from this research survey will be used to develop the State of Ohio’s FMD Emergency Plan, as well as to help identify opportunities for further learning and exploration in FMD preparedness and response.

Below you will find the link and QR code that will take you to the active research survey. This research survey should take approximately 20-30 minutes to complete, and your answers will remain anonymous. Please keep in mind we are NOT gathering any personal information (i.e. names, contact information) or specific details about the locations where the animals are housed (i.e. number of animals on the premises, exact address).
