To request a new project in REDCap, complete the project request form at http://go.osu.edu/newprojectrequest. Please submit a separate request for each separate project.
If you are requesting a Quality Improvement project, please complete a Data Quality Release Form before submitting the project request as you will be asked to upload a copy of the Quality Release approval to the request form. The project cannot be set up until the release form is submitted.
A REDCap team member will reach out to you within 72 hours of receiving the completed request.
