External Surveys
At this time we are requesting that surveys do not go to patients and/or their family without being vetted and approved through the Qualtrics Review Team. Completing the request for an account upgrade will provide the information needed to route the survey for review.
Internal Surveys
If you intend to send a survey to staff nurses, the survey needs to be reviewed by Richard Lucas Richard.Lucas@osumc.edu for OSUNO/ONA approval. It is the consensus of the OSUNO executive Board, Labor Management Council, and HR to send surveys for review to someone from OSUNO/ONA and HR for approval prior to distributing to nurses for transparency and as a courtesy.
PHI
Surveys containing Protected Health Information (PHI) need to be reviewed by Data Security for approval prior to being distributed.
