At the last HackOHI/O meeting, I asked for suggestions/approval for the survey questions, making sure to note that only a few changes could be made due to previous IRB approval. Danny Dotson made a few suggestions and refinements to both Surveys, below:

Exit Q7, about how helpful the Mentors were:
Could you use a Likert scale and give an option that they didn’t use one AND if they didn’t – ask a followup of why. The followup could include “I didn’t know there were mentors available” and things like that to get at why they didn’t visit.
