Patient Case Information

Mrs. L.L. is a Caucasian, 42 year-old diabetic female who presented to the emergency department complaining of upper abdominal pain and chest pain. Mrs. L.L states that she started feeling nauseous and short of breath after her dinner at a local Mexican restaurant.

With the information gathered, the primary care providers ordered the following tests:

  • An electrocardiogram (ECG)
    • test result: in elevated ST segments
    • an echocardiogram that did not show ischemia
    • test result: no ischemia noted
  • Abdominal ultrasounds
    • test result: enlargement of the pancreas
  • Lab work ordered and collected with the following results:
    • Troponin: elevated levels
    • Hematocrit and Hemoglobin: decreased levels (35% and 11)
    • Platelet Count: elevated (600)
    • H. Pylori antibody test: negative
    • H. Pylori breath test: negative

Past Medical History

    • Type 2 diabetes diagnosed at 30 years old
    • Obesity Class I diagnosed at 26 years old
    • Peripheral artery disease diagnosed at 35 years old
    • Hypertension diagnosed at 26 years old

Past Surgical History

    • Hysterectomy performed at 40 years old
    • Wisdom teeth removal at 16 years old
    • Tonsillectomy at 7 years old

Pertinent Family History

    • Diabetes: mother, father, brother, daughter
    • Hypertension: paternal grandfather, paternal grandmother, and father

Pertinent Social History

    • Works as a secretary at a law firm in Columbus, Ohio
    • Lives with her husband and 2 dogs
    • Her hobbies include gardening, reading, and playing cards.
    • Drink alcohol regularly, 3 glasses of wine per day
