Patient Case Presentation

 

Mr. G is a 78 year-old retired school teacher. He currently lives downtown with his wife of 50 years and their 6 cats. Mr. G quit smoking 20 years ago and has been previously diagnosed with hypertension, type II diabetes, and hyperlipidemia. He lives a fairly normal life and his medications have had his other diagnosis under control. Mr. G started to dog sit their daughter’s dog a few times a month. He has been having progressively worsening pain in his legs while walking the dog. He attempted stretching and using a foam roller but there was little or no benefit from them and he was still unable to walk for extended periods of time.  He has had to cut walks short and most recently the pain became too bad for him to continue and his wife had to come pick him and the dog up. 

 

Mr. G gave into his wife’s persistence and visited the PCP. At his visit, the physician inquired about his symptoms as well as taking a look at his family history. The physician checked the pulses in his legs. After palpating weak pulses in Mr. G’s legs the physician checked a blood pressure in his arm vs. his leg and calculated his Ankle Brachial Index. Mr. G’s score was 0.6 which confirmed diagnosis. 

 

Past Medical and Surgical History

Appendectomy, 14 years old.

Diagnosed with Type two Diabetes at 37 years old.

Diagnosed with Hypertension and Hyperlipidemia at 51 years old.

Three broken ribs and a broken nose. Car crash, 43 years old.

Heart attack with stent placement at 58 years old.

 

Family History

Father, Deceased, died of heart attack at 57 years old. 

Mother, Deceased, died of sepsis after a Below-the-knee amputation, secondary to type 2 diabetes 55 years old.

Sister, Alive, healthy 72 years old

Brother 64, Alive, history of stroke 10 years ago. Received TPA in time and has minimal deficits.

 

Social History

Retired High School Math teacher of 55 years

Hobbies: Home Beer Brewing, Fishing, Knitting cat clothes, traveling to visit 7 children, walking 2-3 times a week (before dog sitting), Smoker until 58 years old
