request an upgraded Qualtrics account

An upgraded Qualtrics account through College of Medicine allows users to distribute surveys for educational or administrative purposes. It is not approved for PHI data.

To request an upgraded account:

1. Set up a default Qualtrics account using your OSU Username (name.#), not your Medical Center Username.
2. Complete the Institutional Data Policy training on BuckeyeLearn for the current fiscal year.
3. Complete the request form for an upgraded Qualtrics account.
   • Sign in to the ServiceNow self-service portal.
   • Navigate to the request forms using the following path: Order Services > Access Management and choose “Qualtrics Request.”
   • Under the User Information section, fill in your name.# and other fields will auto-populate.
   • Under the Access Information section, choose “Add New/Update User” as the Request Type.
   • Select “OSU Standard – Accessible” as the Account Type.
   • Select the Division (group) that you belong to and provide a business justification for this request.
   • When the form is complete, click “Order Now” to finalize the request. Click “Checkout” to complete your request.
