If you don’t want to participate
If you do not want to participate in the Coronavirus Study, please complete this form, or contact us via email ocap@osu.edu, or phone/text (614)-407-5737.
If you contacted us to say that you do not want to participate, and then got another letter from the project, it could be because the letter was mailed before we removed your address from our list. Even if you do receive a letter after opting out of the study, we want you to know that we did get your message and the study team will not come to your house.
