Medication Reconciliation Trainers: Overview & Activities

Students complete medication reconciliation rotations at The Ohio State University Wexner Medical Center (OSUWMC), a top-ranked academic health center. Collection and documentation of the home medication list and admission medication reconciliation is an interprofessional process at OSUWMC designed to decrease potential adverse drug adverse drug events that may occur during transitions in medical care. Through these rotation experiences, students enhance their verbal and written communication skills, gain experience reviewing patients in IHIS, and practice problem-solving and patient advocacy. Students apply their didactic knowledge to directly engage in patient care and act as valued interprofessional healthcare team members.

As part of the rotation block at OSUWMC, APPE students will participate in the student medication reconciliation program by training second-year Doctor of Pharmacy students, many of whom have never previously completed a rotation in the hospital setting. This is an opportunity for APPE students to teach and participate as preceptors in layered learning, an experience that most do not have until entry to practice or residency.

Expectations

APPE students must arrive on time, be prepared, and be engaged in rotation. They are to lead by example and maintain professionalism in all interactions with patients, families, preceptors, peers, and other members of the healthcare team. Student groups are responsible for completing a minimum of 2-3 high-quality medication histories.

  • Expectations for training students in their first shift:
    • The APPE student completes the first medication history while the IPPE student(s) observes.
    • The IPPE student(s) completes the second medication history while the APPE student guides.
    • If time allows, the group completes a third medication history.
  • Expectations for training students in their second shift:
    • The IPPE student(s) completes all assigned medication histories while the APPE student observes the patient/caregiver interviews and guides through documentation.

Grading

For this program to be successful, you will need to be prepared. If you do not perform well in this activity, your APPE grade for the month will be affected. Your preceptor will be notified to ensure awareness of your responsibilities, required training, and rotation time commitments.

Dress and Behavior Code

Pharmacy students are required to maintain professionalism in appearance and behavior. They must be clean and well-groomed and wear an OSUWMC-issued hospital ID badge and a short white coat while in patient care areas.
