CURRENT RESEARCH INTERESTS
My current research interests are in the optimization of chemo radiation strategies, development of radiation measurement systems in biology and medicine, development of clinical treatment planning methods and development of clinical trials in the department.
Over the past few years my clinical research interests are in developing and standardizing QA processes and programs. Our department currently has ten truebeam accelerators, which we planned and executed the commission for as dosimetrically equivalent and beam matched machines. My group has developed QA data repositories that allow us to compare and dashboard data from all our linacs and detect trends and variations between machines. Recently, I have been part of a core team that designed and is implementing a quality, operations and outcomes database repository within the department.
I have an interdisciplinary background with a Masters and PhD in Biomedical Engineering, and I am a category M faculty in the Nuclear Engineering Graduate program, and on the Medical Applications track. I currently advise graduate students in Nuclear Engineering for their masters projects as needed and a part of the Medical Applications research group in Nuclear Engineering, and medical students and medical and medical physics residents in various dosimetry projects.
In the past I have been involved with research in Boron Neutron Capture Therapy (BNCT) physics, mixed field dosimetry. I have extensively used neutron/photon/ electron Monte Carlo Transport codes to model the moderator (filter) assembly for the accelerator-based epithermal neutron source for BNCT and come up with a suitable design for fabrication and to predict the dosimetry in head phantoms. I have fabricated the first complete moderator assembly prototype, which is still used in the neutron beam research here at OSU. I also conduct research in modeling the different dose components in a mixed radiation field and used them to develop a rudimentary treatment planning system for BNCT with an algorithm to analyze brain tolerance from the mixed radiation field. This tool has been used extensively in our research for designing better neutron beams for better performance for deep-seated tumors, and participated in designing and conducting clinical trials around drug delivery and resulting dosimetry.
The above research interests have evolved into evaluation and design of combination therapies that involve radiation therapy. I have subsequently been involved with several collaborators in conducting small animal studies to evaluate combination therapies and optimizing drug and radiation delivery times.
My other clinical research interests are in state of the art radiotherapy delivery methods and objective ways of creating an image and knowledge-guided target for Radiosurgery, 3D Treatment Planning, Conformal Radiation Therapy, and Intensity Modulated Radiation Therapy. We have developed a Nodal Atlas as a tool for target definition in Radiation Oncology. This nodal atlas would aid the radiation oncologist in objectively determining the clinical target volume based on the gross target volume and clinical information like staging. We have produced an interactive CD-based 2D Nodal Atlas utilizing Adobe Acrobat, which will be computer platform-independent. However, a key use of the cross-sectional nodal atlas would be to have it available as a reconstructed 3D atlas. Visualization of nodal stations in 3D, as well as their relationships with surrounding normal tissues, will help the radiation oncologist to determine the optimal beam design in a BEV (Beams Eye-View) environment. Various levels of intelligence can then be built in to this atlas, like tumor staging and nodal involvement probabilities to expand this tool. This would provide a combined tool for the radiation oncologist to weight the complication risk vs. benefit of tumor control in planning complex treatments such as conformal and intensity modulated radiotherapy.
I have actively participated in collaborating with several clinical faculty in designing and developing clinical trials for radiation treatments in institutional trials, and helped open many national clinical trials by completion of the credentialing and a data submission infrastructure in the department.
CLINICAL EXPERIENCE/ INTERESTS
I have more than twenty years of experience in clinical medical physics, and have commissioned more than fifteen linear accelerators, multiple treatment planning systems and other radiotherapy equipment. I have introduced every modern radiotherapy technique in the clinic. In my leadership role as a chief of clinical medical physics I have been involved with strategic planning for departmental programs, recruitment, and design, construction, equipment installation planning and clinical go live for three new cancer center buildings, including our Proton Therapy center, and an outreach cancer center. I have also led the development and expansion of our department IT infrastructure to enable the rapid expansion and support of the Radiation Oncology Department, including the strong portfolio of innovative software tools to enhance operations, like multiple whiteboards for management of clinical workflows and numerous automated scripts for automation of quality and safety operations. I have recruited over 30 highly talented faculty and staff physicists into the James Cancer Hospital to build a strong medical physics program to support our academic, clinical, and teaching mission areas.

- Teaching for Residency Program
- Radiation Physics: I have team taught the Radiation Physics class for our Radiation Oncology Residency Program since my hire in 1998. I have taught anywhere between 25% to currently about 10% of this class. I have been the course director for this course since 2007
- Radiation Physics Didactic Course for PGY IV Residents: Me and one other colleague Jeff teach this yearlong in-depth physics review course meeting 1 hr per week with our PGY4 residents to prepare them for their Physics Boards. I teach about 50% of this course and Jeff and I have created this curriculum and teach it using Carmen since 2018. This course has typically 2 to 3 PGY4 residents and since we have started this course the physics board pass rate has been 100%.
- Radiation Biology: I have taught two lectures for the Radiobiology didactic series since its inception.

Curriculum Development
- Annual Symposium: For the past two years I have co-directed a National Symposium that the department has hosted as part of increasing the national recognition for the Radiation Oncology International Training Center. Our 2017 symposium was on “Innovations in Immobilization and Patient Setup”, with an audience of about 100 Radiation Therapists, Radiation Dosimetrists, Medical Physicists, Radiation Oncologists and Radiation Oncology Vendors. Our 2018 symposium was on “Innovations in Program Development and Delivery Technologies for Stereotactic Body Radiosurgery” for a similar audience and attended by about 100 attendees as well. I have dedicated a lot of effort with my co-director, our organizing committee, and the OSU James-CCC conference management team to develop curriculum, identify speakers, organize hands-on labs, getting continuing education credits approved for each of the attendee types, and executing the conference.
- For the past three years (2015 – present) I have worked closely with department leadership to lead the development of curriculum and hands-on training programs for several professional training programs as part of the Department of Radiation Oncology International Training Center, and seen them through execution and continuation of development of curriculum. The Radiation Oncology International Training Center is a unique facility that houses the only non-clinical Linear Accelerator in the nation dedicated for clinical training within clinical spaces. So far the courses that have been developed and are being taught on a regular basis are: a) Introduction to Radiation Oncology – a two day introductory course for members of the radiation oncology industry; b) Optical Surface Monitoring Systems – a two day course on commissioning, QA and implementing a clinical program around optical tracking systems in advanced radiotherapy practice; c) Clinical Truebeam Commissioning – a three day course on expedited and beam matched commissioning of multiple linacs and development of a robust QA program for a multi-linac department. Courses under development include a) VMAT and Portal Dosimetry commissioning school; b) Imaging based Brachytherapy planning, QA and delivery clinical school; and c) Prone Breast Radiotherapy clinical school.
- Over the past decade I have developed and implemented a Medical Physics Clinical Residency Education Program for Radiation Oncology at the James Cancer Hospital. This is a two-year residency program integrated with Graduate Medical Education program at The Ohio State University, College of Medicine. We are currently funded for four positions by the James Cancer Hospital and GME. The residency program was accredited by the Commission on Accreditation of Medical Physics Educational Programs in 2012 initially for five years, and we have been re-accredited in 2017 for another five years. The program has constantly evolved and developed over the past years. Accomplishments include a) I have implementing a customized version of a commercial web based competency tracking and evaluation software for electronically managing the residency program, b) introduced an application database to manage, triage, and track applications coming into the program, as well as automate the correspondence process to candidates, c) introduced clickers for residency teaching, d) introduced a very rigorous annual curriculum review and change process, which includes a program evaluation by all residents and faculty and identification of strengths and areas to improve which is reviewed and managed and acted upon by our curriculum review committee. In 2015 we completed an agreement with the US Armed Services to host one medical physics resident every other year from the armed services. The Medical Physics Residency Program has graduated its first resident in June 2005, and has graduated 18 residents thus far.
- I have been the course director for the Physics Course for the Radiation Oncology Residency Program since 2007. I have helped evolve the class constantly to streamline the education of our residents. I introduced clickers into our teaching and assessment of learning for the residents.
- In 2008, the Radiation Therapy Program was converted from a 1 Year certificate program accredited by the Joint Committee on Education in Radiologic Technology, offered by the James Cancer Hospital and Solove Research Institute into a Bachelors program through the College of Allied Medical Professions. During this process I worked with the program director to re-design the radiation physics, brachytherapy, and Quality Assurance portion of the didactic coursework, create content and the core course requirements, which lead to the creation of the following courses RASCI 481, RASCI 482, RADSCI 530, and RADSCI 595. I have taught a majority of these courses since their creation. These courses and their content are being further developed changed since last year as part of the semester migration of the program. As part of this process I have worked with the program director and program coordinator to evaluate and change the Radiation Physics course content and the text book this year. In 2012, with OSU migrating to the Semester system, I participated in re-mapping all the physics classes and content into the Semester calendar.
- Starting Fall 2001 I started teaching Radiology 670 (Radiation Therapy Physics). This course was not taught for several years. I re-designed the curriculum, to change the scope and depth of teaching, since the intended audience of the course has changed over the years. The interest of students in learning Radiation Physics come either from engineering students like Nuclear Engineering which has a Medical Applications track that include Medical Physics and Health Physics. I am a graduate faculty in Nuclear Engineering, and am in their Medical Applications Faculty Core. The other group of students interested in this course are students in biological sciences who use/intend to use radiation for their research and want to learn more about the physics of therapeutic radiation beams. I have redesigned the course to cater to the need of both these groups. I have incorporated handouts and other materials to better cover the specific needs all the backgrounds.
- Since my hire in this department I have developed and taught two courses Brachytherapy and Computer Theory for the Radiation Therapy Program. Brachytherapy was a previously taught course, which I have re-designed to make it completely based on a teaching packet that I hand out. This was done to tailor the class completely to the needs of the students. Computer Theory was a class that was introduced by me at the request of the Program Director. This 1 credit course and its curriculum was designed to teach the students the basics of computers and more importantly to teach the basics of the electronic charting environment that we are in today, including databases, networking and the worldwide web. In order to make the class more hands on and appealing to students I have introduces over the years several projects and labs that give them a hands-on exposure to computers in radiation oncology. Both these classes have now been absorbed in to the physics classes taught as part of the Semester curriculum in 2012.
- In 2004 I have developed with one of my clinical colleagues a series of lectures to constitute a Professional Brachytherapy Workshop. The workshop consists of closely coordinated series of didactic lectures on the clinical and physics/dosimetry aspects of Brachytherapy practice for different sites as well as lectures on basic brachytherapy physics and quality assurance. The intended audience for these lectures is Radiation Oncologists and Medical Physicists who would like to get trained in the clinical use Brachytherapy. The workshop was intended to be delivered either as a two or three day didactic workshop, or a one week didactic and hands-on workshop. The first joint clinical/physics workshop was given in Feb 2004 by Dr. Subir Nag and me at Singapore as part of the Singapore Radiological Society Meeting. The second workshop was funded by the IAEA and held in Peru in Dec 2004, and the physics lectures were given by one of my colleagues. The third workshop was commercially funded and held in Kolkata, India, in November 2007 and was taught by Dr. Nag and myself.
