Immunizations

Submission Instructions: All vaccination records must be submitted directly to Wilce Health Center through MyBuckMD by the end of first semester of enrollment. The flu vaccine will be due November 1st.

  • All immunizations, titers, and TB testing must be complete 30 days prior to the start of the program.  To comply with this requirement, complete this form and upload to MyBuckMD.

BCI and FBI Background Checks

Submission Instructions: Please click here for instructions and the reason code to complete the background check requirement.  The results must be sent directly to OHR by end of first semester of enrollment in nursing coursework.

  • This can also be fulfilled if the student completes the BCI& I and FBI background check requirement for CEHE.

 

CPR Certification

Due Date: by the end of the first semester of enrollment

Submission Instructions: submitted to College of Nursing Student Affairs

  • The College of Nursing only accepts BLS for the Health Care Provider or BLS Provider from the American Heart Association.
  • Please have the front and back images of the student’s CPR card on one page in PDF format.  This certification expires every 2 years and must be updated with the College before the expiration date to remain compliant.

Drug Screenings

Submission Instructions: completed at or submitted to Wilce Student Health Center

Due Date: Assigned dates by Student Affairs during first semester. Drug screens can be completed between 8:30am and 4:30pm. No drug screens will be ordered past 4:30pm.

  • Students are able to  complete this requirement free of charge at Wilce Student Health Center.   ** Please bring prescriptions of all medications/supplements currently being taken in the case of a false positive result. **
  • All College of Nursing students are required to complete a drug screen. Students will not be permitted to attend clinical or health assessment if results have not been received.
  • If you are an online distance student you may have your our drug screen completed at another location, but the cost will not be covered by the College of Nursing.  Please see these directions  in order to be screened for the correct substances.  Insufficient panels will not be accepted.
  • You may use this form to also document your drug screening.

 

HIPAA:

All College of Nursing students are required annually to complete HIPAA certification.

This form is to be signed by all students and faculty accessing patient information or providing patient care. This may require access to computerized patient record information such as IHIS (Integrated Healthcare Information System); One Source; Pyxis medication access; getting patient care supplies and equipment accessed through Omnicell; bar-coded ID access to specialized units or areas such as Women & Infant and entrance to secure medication and patient care supply areas for individual units.  This form can be signed electronically at the link below.

Release of information authorization form

The Ohio State University College of Nursing clinical sites are obtained throughout Franklin and contiguous counties. When the OSU Medical Center (Wexner Medical Center, Ross Heart Hospital, James Cancer Hospital, Harding Hospital and OSU East Hospital) the OhioHealth System (Riverside Methodist Hospital, Grant Medical Center, Doctor’s Hospital, Grady Memorial and Dublin Methodist Hospital) and Mount Carmel Hospital System (Mount Carmel West, Mount Carmel East and Mount Carmel St. Ann’s) are utilized it is a necessity for students to have access to computer systems used for clinical care. If a student does not have access to the computer system, the student will not be able to meet the requirements of the clinical course and may be at risk of failing the course. The above hospital systems where the college of nursing places a student for clinical experiences require that a unique identifier such as a social security number or ITIN (individual tax identification number) be used for each student in order to create access to these systems. OSU Medical Center also requires date of birth. Signing this form grants The Ohio State University College of Nursing permission to release this information to any of the above institutions in the event you are placed in one of these facilities. HRSA Survey

College of Nursing Badges

The College of Nursing requires all graduate level students to wear the College of Nursing badge for identification purposes in clinical or assessment settings.  The College will cover the cost of the initial College of Nursing badge.  In order for the badge to be printed, you must have a photo on file with the BuckID Office.  Please refer to the following instructions in order to receive a badge.

Instructions for On Campus Students:

     You must go to the BuckID office to have a photo taken to have your College of Nursing badge made there the first 2 weeks of class.

                Hours of Operation:       8:00 a.m. – 5:00 p.m. Monday through Friday

Instructions for Online Students:

  1. Visit myphoto.osu.edu after July 1st to upload a photo to begin the process of printing your College of Nursing badge by August 1. You must upload a photograph even though you may have been issued a BuckID previously.
  2. You will be required to submit the following:
  • a passport-style photo (well lit, neutral background, etc.)
  • a picture of their gov’t-issued ID – typically a driver license, and
  • signature – you will sign electronically
  1. Update or confirm your home/current address on Buckeyelink.

You will receive your College of Nursing badge upon completing the steps outlined above.  Our office will send the badge to your current/home address listed in Buckeyelink.

In addition to the College of Nursing badge, graduate students who are/will be completing clinical experiences in any area of the Wexner Medical Center system also will be required by the Medical Center to obtain a Medical Center badge which allows students access to secured areas of the hospital and are generated by the Medical Center Security ID Processing Office. Information about Medical Center badges will be sent at a later date directly to students who are required to have one.

College of Education and Human Ecology Requirements

Due 30 days prior to start of term you are enrolled in Nursing 3118

 

The following items are required by the College of Education and Human Ecology. CEHE requirements must be submitted here: edprep@osu.edu

CEHE requirements include:

  • BCI&I/FBI background check (See below for additional information on how to complete this requirement)
  • Practicum Application Form
  • Minors Training Video Certificate:
    • Go to BuckeyeLearn and log in
    • Click “Browse Training” on the left hand side of page
    • Scroll down until the “Activities and Programs with Minor Participants Training” is visible.
    • At the completion of video, there is a quiz. You will be required to save AND/OR print the certificate of completion.  Submit the certificate at edprep@osu.edu
  • Standards of Behavior Form

 

The CEHE background check can be completed in one of two ways:

  1. Complete the BCI&I/FBI background check at the Office of Human Resources.  The following codes must be used to fulfill this requirement:
  • BCI&I background check with the code of 3319.39 (B3)
  • FBI background check with the code of 3319.39
  • Complete this form prior to being finger printed and indicate the results must be sent to the following campus address:

  • If you plan to apply for licensure within the year, you must request a copy of the background checks to be sent to the Ohio Department of Education.  You can indicate this as your preference on the above form.

Background checks are valid for one calendar year from the date they are completed

Or

  1. If the student is placed in a practicum experience at his/her employer, the student may submit a letter from the employer’s Human Resource department that verifies:
  2. The employer will take responsibility for the student during his/her practicum experience
  3. The employer will verify the student has completed a BCI&FBI background check for employment purposes

If a student is seeking licensure at the completion of the certificate, the Ohio Department of Education will require all applicants to have a current background check (within one year) on file with their office.
