School Nurse Professional Pupil Services Licensure Certificate

As of today, May 15, the College of Nursing will resume the tracking of compliance requirements. Non-compliant and at-risk students will receive notifications weekly. In addition to the notifications, you can view your compliance status through the Student Portal.  Please contact our office by email if you have any compliance-related questions.


To complete the background check requirement:  visit an agency in your area offering fingerprinting services.  Please visit the Ohio Attorney General’s website to locate an agency nearest you. It’s advised to call ahead before traveling to any location to verify if they are still providing fingerprinting as service availability across the state has been changing daily. Please submit results to:

To complete the vaccination requirements and drug screen: 

The Wilce Student Health Center remains open during the stay-at-home orders.  They offer vaccination services and drug screenings on the following days and times:

Health professional student vaccine/titer/TB testing:

Monday – Friday from 8:00am – 4:00pm (appointment required)

Walk-in drug screens:

Monday – Friday from 9:00am – 4:00pm (no appointment necessary)

CPR:

The American Heart Association (AHA) has granted a 120-day extension to expiring BLS Provider certifications.  The extension has been applied to deadlines that were set to expire in the months of March-June. If students do not renew their CPR certification during the extension, they will be marked non-compliant at the end of the 120-day time period.  The college will revisit this policy as needed as the situation with COVID-19 progresses and more information is available.


The program compliance requirements for the School Nurse Professional Pupil Services Licensure Certificate program are listed below along with deadlines and submission instructions.  Requirements are broken down into College of Nursing items and College of Education and Human Ecology items.  Once you have enrolled in nursing coursework, you will be able to view you compliance status at any time by logging onto the Student Portal with your name.# and password.

College of Nursing Requirements

RN License

Deadline: Start of the Certificate Program

You are required to submit your RN License at the start of the program. Please follow these instructions when submitting the required documentation. You must upload a summary of your license including your full name, license number, and expiration date.  A screenshot of your license can be taken from the Board of Nursing website. The document must be saved as a PDF file.

Immunization Requirements

Deadline: November 1

You are required to be in progress of completing the following immunizations.

  • Hepatitis B
  • MMR
  • Varicella
  • Tetanus
  • Two Step PPD
  • Influenza

A description of each immunization is listed here.  All immunizations are due November 1.  To comply with the immunization requirements, complete this form and upload the document to MyBuckMD.  Be sure to select the ‘Health Professional Compliance’ tag to avoid processing delays.


BCI and FBI Background Checks Requirement

  • This can be fulfilled if the student completes the BCI and FBI background check requirement for CEHE listed below.

CPR Certification Requirement

Deadline: November 1

You are required to complete a BLS Provider CPR certification course through the American Heart Association.  No other CPR certification will be accepted by the College of Nursing.

You must submit appropriate documentation to the College of Nursing Office of Student Affairs, Equity and Inclusion by November 1.  See sample documentation here.

  • CPR documentation can be submitted here.
  • This certification expires every two years.  Re-certification documentation must be submitted to the college before the expiration date to remain compliant.

Drug Screenings

Deadline: November 1

All College of Nursing students are required to complete a drug screen. You will not be permitted to attend clinical or health assessment if results have not been received.  Drug screens can be completed between 8:30am and 4:30pm.  No drug screens will be ordered past 4:30pm.

If you are an online distance student, you may have your own drug screen completed at another location, but the cost will not be covered by the College of Nursing.  Please see these directions in order to be screened for the correct substance.  Insufficient panels will not be accepted.

  • Drug screens completed at the Wilce Student Health Center are free of charge.
  • Please bring prescriptions of all medications/supplements currently being taken in the case of a false positive result.

HIPAA

All College of Nursing students are required annually to complete HIPAA certification.  See FAQs here.

  1. Login to https://buckeyelearn.osu.edu  .  If you have not previously affiliated with OSU and experiencing login issue, please try again when you are eligible to enroll for classes.

  2. Click “My Transcript” button and locate the HIPAA training and click “Launch”.  If you have not previously affiliated with OSU and HIPAA training is not listed, please try again when you are eligible to enroll for classes.

BCI and FBI Background Checks Requirement

Deadline: November 1

The CEHE background check can be completed in one of two ways:

1. Complete the BCI/FBI background check at the Office of Human Resources.  The following codes must be used to fulfill this requirement:

  • BCI background check with the code of 3319.39 (B3)
  • FBI background check with the code of 3319.39
  • Complete this form prior to being finger printed and indicate the results must be sent to the following campus address:

  • If you plan to apply for licensure within the year, you must request a copy of the background checks be sent to the Ohio Department of Education.  You can indicate this as your preference on the above form.

Background checks are valid for one calendar year from the date they are completed

 

2. If the student is placed in a practicum experience at his/her employer, the student may submit a letter from the employer’s Human Resource department to edprep@osu.edu

  • The letter must be on their letterhead from someone with authority (usually this is HR)
  • The letter must contain information on which background checks were completed and the expiration dates
  • The employer must indicate that the background checks satisfy their requirements for the placement

If a student is seeking licensure at the completion of the certificate, the Ohio Department of Education will require all applicants to have a current background check (within one year) on file with their office.

More information will be shared with you about the background check during the first nursing class.  Please do not complete the background check requirement until your instructor provides more detail.
