Earn 2019 Premium Credit
If you plan to enroll in an Ohio State medical plan for 2019, you may receive a premium credit to reduce the cost of your employee medical plan contributions with the completion of the Personal Health and Well-being Assessment (PHA) and a verified Biometric Screening. Enrolled faculty and staff can receive $20/month by completing both requirements, and $30/month if their enrolled spouse does as well. Complete both steps below in 2018 to lock in receiving the full premium credit for 2019.
- Visit your primary care provider and submit a provider fax form to the OSU Health Plan.
- Schedule a complimentary on-campus biometric screening.
If you have not already registered for the new wellness portal through Virgin Pulse, you will need to do so prior to completing the PHA. For help with registration, visit the Quick Start Guide for step-by-step instructions.
