The State 4-H Office requires all 4-H Clubs to submit their annual 4-H Club Treasury Reports electronically. The form requires your club’s bank name, EIN, account number, amount and itemized listing and description of all income and expenses for the year. You will want to reference your account register and/or bank statements in completing the report.
Because the State 4-H Office is requiring these forms to be submitted electronically—we are encouraging organizational advisors to complete the forms online and submit them yourself. However, we recognize that the electronic form, directions, and submission process are fairly complex, especially if you do not have a lot of computer skills or have access to a computer and the internet.
If you need help with completing this required report, please contact the Wayne County Extension Office for assistance. This form must be completed and submitted by January 31st or your club will not be permitted to operate in 2015.

You will need to have the latest version of Adobe Reader downloaded on your computer for the file to open.
